If your child has asthma, a food allergy or anaphylaxis, we need a plan from your doctor before they start. Download the right one below, take it to your GP to fill in and sign, and bring it in on the first day.

  From ASCIA, for children prescribed an adrenaline device.

• Allergic reaction action plan (opens in a new tab)

  From ASCIA, for children with a food or insect allergy who have not been prescribed an adrenaline device.
